What you need to bring:
Valid Photocard ID (Passport or Driving Licence)
Form E and Council Medical Report - downloads below
Completed questionnaire- download below
Glasses if Worn (and your latest prescription strength)
Contact Lenses if Worn (they will have to be removed during the examination)
The Council Prefers for You to See Your Own GP for the Medical - You must be registered at Spencer Street Surgery to have your medical with us. A number of GP surgeries are not carrying out examinations currently, we are happy to do these but this needs to be OK'd by the licencing dept. first.
To do this we need sight of your medical record. If you ask a receptionist at your GP surgery to print out or email you a 'brief summary' (advise them to go to consultations, click print, select brief summary if they don't know how to do this)
This medical brief summary is really important- if we cant sign that we have seen this the licencing dept. wont accept your medical.
If you do not bring this summary to the appointment and require us to complete the medical later after sending the summary through the cost increases from £55 to £100 due to the additional time taken.
