What you need to bring:
Valid Photocard ID (Passport or Driving Licence)
Taxi Medical Form...download below
Glasses if Worn (and your latest prescription strength)
Contact Lenses if Worn (they will have to be removed during the examination)
The Council Prefers for You to See Your Own GP for the Medical - Ideally you will be registered at Spencer Street Surgery to have your medical with us. As a number of GP surgeries are not carrying out examinations currently, we are happy to do these, this has been OK'd by the licencing dept.
To do this we need sight of your medical record. If you ask a receptionist at your GP surgery to print out or email you a 'brief summary' (advise them to go to consultations, click print, select brief summary if they don't know how to do this) It may be possible to access a medical summary online- see below.
This medical brief summary is really important- if we cant sign that we have seen this the licencing dept. wont accept your medical.
If you do not bring this summary to the appointment and require us to complete the medical later after sending the summary through the cost increases from £55 to £100 due to the additional time taken.
iT IS SOMETIMES POSSIBLE FOR US TO ACCESS A MEDICAL SUMMARY ONLINE. IF YOU WISH US TO TRY TO DO THIS YOU WILL NEED TO BRING WITH YOU THE MOBILE PHONE REGISTERED TO YOUR NAME ON YOUR NHS RECORD. SOME GP SURGERYS DO NOT HAVE THIS FEATURE ENABLED.
Our first responsibility is to road safety, and making sure that you comply with the law with regards to medical fitness to group II standards. If you are found to be unfit to hold a taxi licence at the medical we will need to inform the licencing authority of this decision. By booking a taxi medical you give us consent to inform them of this outcome.
