Shotgun/ Firearms Licence application

This service is open to patients of spencer street surgery and those registered elsewhere.

It is always best for this examination to be carried out by your own gp who knows you, and has sight of your full medical record, so always ask your own GP surgery first.

If your own surgery isnt carrying these out we are happy to carry out this service for you

Summary of your NHS Records is necessary

We cant carry this out without a current summary of your NHS Medical record.


THIS SUMMARY NEEDS TO GO BACK 20 YEARS OR IT WILL NOT BE ACCEPTED BY THE FIREARMS LICENCING DEPARTMENT

What you need to bring

 You need to bring photocard ID such as a passport or driving licence, the paperwork from firearms licencing and a summary of your nhs medical record.

 If you ask a receptionist at your GP surgery to print out or email you a 'brief summary' (advise them to go to consultations, click print, select brief summary if they don't know how to do this)

This medical brief summary is really important- if we cant sign that we have seen this, we cant proceed.

If you do not bring this summary to the appointment and require us to complete the medical later after sending the summary through the cost increases from £55 to £100 due to the additional time taken.
