Please be sure to register your phone number when you book.
This includes an eye test. If glasses are worn for driving we need to know prescription strength. If contacts, the kit to remove them is needed. If on blood pressure drugs the last 3 blood pressure readings would be helpful. If you are medically complicated a 'brief summary' from your own GP practice would help- no appointment needed, ask the reception to load your file, go to consultations, click on print, select 'brief summary' and either print out or send you a copy by email or SMS. Bring this on the day.
This is the medical examination for medical fitness to drive Group II vehicles.
Please be aware that our primary responsibility here is to road safety and to make sure that you are compliant with the relevant law with regard to being a professional driver. If you do not meet the group II medical standards we will advise you either on how to meet those standards, such as to go to an optician and obtain some glasses, bur if we are very concerned you have a disqualifying condition, we may advise you to stop driving and inform the DVLA. It is your responsibility to do so. If despite that advice we are concerned that you may continue to drive then we are obliged to inform them. By booking a HGV medical you give your consent for us to do so.
Please send your medical and application form back to the DVLA by recorded delivery. If there is delay or mishap and the DVLA have signed for them then all will be well, but if they haven't then there is a risk that if your licence expires then you will have to stop driving whilst any problems are sorted out at your own expense.
If asked to send your photocard please take and retain a photo of front and back before posting it.
